Welcome! To make your first visit smooth and efficient, please have the following ready:
- Insurance Details (if you have insurance): Provider name, member ID, and group number.
- Medications & Health History: A current list of medications and any relevant medical conditions.
- Dental History: Past treatments, surgeries, or areas of concern.
When you schedule online, we’ll send your new patient forms via email, please be sure to fill this in correctly so you can receive these forms.
If you have recent X‑rays (within the last year) from another dental office, email them to SmileLogic@gmail.com ahead of your dental hygiene visit. If they’re over a year old, we’ll discuss whether updated images are needed based on your current oral health.
